Account & Billing Agreement Form
To set up an account with Dunwoody Labs, you must complete the online Account & Billing Agreement.
Once your Clinic Account agreement is reviewed, you will receive an e-mail from SnapSign Notifications stating: Invitation to New Document: Dunwoody Labs E-Signature Request. Inside the e-mail, it will say "Hello, please review and sign the attached document to complete your account agreement with Dunwoody Labs." Please be on the lookout as sometimes it will arrive in your junk folder. After you receive the e-mail for the signature we will send you your welcome e-mail with login information.
